Completing therapy intake forms online before your first session helps your therapist prepare and saves time in the room, and Serene keeps all of it in one place so you are not chasing paperwork across email. This guide shows how to find the forms your therapist assigned, fill them in, and confirm they are done. Everything happens in your client portal, which you reach with the email you booked with.
Your therapist assigns intake forms to you, and they appear in your client portal under the Intake Forms section. To get there, go to the find your portal page, enter the email you booked with, and verify with the short code Serene sends to that email. The Intake Forms section only shows up when you actually have forms to complete, and it disappears once everything is done. So if you do not see it, either your therapist has not assigned anything yet or you have already finished.
Your intake shows up as a checklist, where each item is a form or task your therapist wants completed. Opening a form lets you read and answer its questions, and the questions are whatever your therapist chose, so one practice's new-client form can look quite different from another's.
Work through the questions and answer them as fully as you can. Some questions may be required, and honest, complete answers help your therapist understand your situation before you meet. When you have finished a form, save or submit it, and that item is marked as done on your checklist.
If a form asks you to share a document, such as insurance paperwork or a referral, you can add it through the My Uploads part of your portal. Anything you upload or submit goes only to your therapist.
As you complete items, your checklist keeps track of what is finished and what is still pending, so you can always see at a glance whether anything is left. Once every item is complete, the Intake Forms section clears itself, which is your sign that there is nothing more to do.
Your therapist sees your completed forms on their side, so you do not need to send anything separately or let them know you are done. Submitting the forms is what tells them.
If a form will not submit, check whether you missed a required question, since that is the most common reason a form stays incomplete. If you booked with the wrong email and cannot find your forms, remember your portal is tied to the exact email you used at booking, so try the address on your confirmation.
